Consent Agreement

This Agreement is entered into between Veterinary LinQ LLC, a Texas limited liability company ("Vet LinQ"), and you as a representative of the clinic described above:

Purpose:

The purpose of this Agreement is to authorize Vet LinQ to securely access and retrieve patient medical records from Clinic's electronic medical record (EMR) system for the sole purpose of fulfilling authorized pet owner record requests and improving continuity of care among veterinary service providers.

Authorization of Access:

  1. Clinic grants Vet LinQ permission to access and retrieve any patient medical record contained within Clinic's EMR upon verified owner request through Vet LinQ's platform.
  2. This authorization extends only to data retrieval necessary for the requested record and does not permit Vet LinQ to share, sell, or otherwise distribute Clinic's data to any third party.
  3. Access may be accomplished via secure API, encrypted data transfer, or other mutually agreed technical means.

Data Security & Compliance:

Vet LinQ agrees to maintain industry-standard security and privacy controls, including:

  • End-to-end encryption (AES-256) for all data in transit and at rest.
  • OAuth 2.0-based access tokens and scoped authorization.
  • Zero-trust architecture with least-privilege access.
  • Third-party vulnerability and security audits.

Data Retention & Use

  1. Records accessed will be read-only. Vet LinQ will not alter or allow for any editing of medical records.
  2. Vet LinQ will not store any medical records retrieved from Clinic.
  3. Vet LinQ may retain limited metadata and analytics (e.g., access logs, record hashes, audit entries) solely for security, compliance documentation and to improve performance and delivery of records.

Revocation & Termination

  1. Clinic may revoke this authorization at any time by providing written notice to Vet LinQ at helpdesk@vetlinqsearch.com.
  2. Upon revocation, Vet LinQ will immediately disable access and confirm termination in writing.
  3. Either Party may terminate this Agreement with or without cause upon written notice.

Representations

Clinic represents that the undersigned account manager is duly authorized to execute this Agreement on behalf of the Clinic and to grant the rights described herein.

Vet LinQ represents that it will comply with all applicable laws, ethical standards, and veterinary confidentiality obligations in carrying out the purposes of this Agreement.

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of Texas, without regard to its conflict-of-laws principles.

Entire Agreement

This document constitutes the entire understanding between the Parties and supersedes all prior discussions or agreements concerning the subject matter herein. Any modifications or updates must be made in writing and signed by both Parties.
